Bias Incident Report Form

This form is to report incidents of bias or discrimination within the Town of Danvers. Please complete the following form if you feel your Civil Rights have been violated. For immediate crimes or emergencies, contact 911 or local police.

To report a Hate Crime, go to the Department of Justice website.

Information here will be tracked by the Director of Equity and Inclusion to identify community trends and connect individuals to resources.

Hate Crime vs. Bias/Hate Incident

According to the Department of Justice, at the federal level, a hate crime is a crime motivated by bias against race, color, religion, national origin, sexual orientation, gender, gender identity, or disability.

Bias/hate incidents are hostile expressions motivated by bias against race, color, religion, national origin, sexual orientation, gender, gender identity, or disability, that contribute to feeling unsafe or unwelcome. (Adapted from the Oregon DOJ.)
