Danvers Office of Veterans' Services Financial Assistance Application (MGL Ch 115)
This application is for financial assistance based on Massachusetts General Law Chapter 115.
This is information based on Veteran's service. This information will be verified by the Veteran's DD214. If you are the Veteran, please skip to BRANCH OF SERVICE
If you are married or have minor children, please list them below. You will need to provide their ages, social security numbers and relationship. You will also be required to provide birth certificates and marriage certificates as well. If you are a widowed spouse, you will have to provide a copy of the death certificate for your Veteran spouse.
Please submit a copy (if applicable) of your mortgage agreement or rental agreement (lease). If you do not have that, please submit a copy of recent (within the last 30 days) of a bill with your name and address on it.
Number of vehicles in household. Please list the year, make, model, registration number and state of each vehicle. List all vehicles even if not registered
by submitting this application for benefits, I attest that the information I have provided is honest and accurate to the best of my ability.
